Peptides
Tesamorelin + Ipamorelin: two GH signals, no tested blend
How does it work?
Tesamorelin stimulates the GHRH receptor; ipamorelin is a growth-hormone secretagogue acting through a different receptor. The idea that both signals could raise growth hormone is biologically plausible, but a stronger hormone response does not by itself establish more fat loss or better recovery. [1,2]
What do studies show?
Prescription tesamorelin has clinical evidence for reducing excess abdominal fat in adults with HIV-associated lipodystrophy. That narrow indication is not general weight loss. Ipamorelin has separate, limited human research. We found no randomized human outcome study of the exact tesamorelin-plus-ipamorelin vial, so there is no evidence that adding ipamorelin improves tesamorelin’s benefits or safety. [1,2]
Safety
The prescription tesamorelin label discusses glucose intolerance, IGF-1 elevation, and injection reactions. Adding an unapproved secretagogue could change effects and risks. The approved Egrifta WR product is a specific 11.6 mg-per-vial formulation; its instructions explicitly apply only to that formulation and cannot validate a research blend. [1]

EVIDENCE CONTEXT
Why?
The cited evidence does not establish a dose for this use or product.
Egrifta WR’s labeled regimen is 1.28 mg tesamorelin subcutaneously once daily for its defined indication and formulation. It contains no ipamorelin. The catalog blend is labeled 10 mg tesamorelin plus 5 mg ipamorelin per vial, which is inventory rather than a studied dose. No effective or safe exact-blend regimen has been established, and component doses should not be combined into a fabricated recommendation. [1]
